肠道菌群与胆汁酸谱的双向调控机制

摘要:肠道菌群与胆汁酸共同构成肠-肝轴中的重要调控网络。胆汁酸在肝脏中合成后进入肠道,在肠道菌群作用下经历去结合、脱羟基、氧化和异构化等过程,形成具有不同功能活性和生物学效应的胆汁酸谱。反过来,胆汁酸又通过其抗菌特性、受体信号及对肠屏障的调节作用影响肠道菌群组成与功能。近年来,围绕肠道菌群-胆汁酸轴的研究不断深入,提示该轴异常不仅参与代谢相关脂肪性肝病、酒精相关肝病、病毒相关肝病、胆汁淤积性肝病及肝细胞癌的发生发展,还与炎症活动、纤维化程度、治疗应答及预后密切相关。胆汁酸谱及菌群特征的联合分析有望为肝病的无创分层、动态监测和精准干预提供新的思路。本文围绕肠道菌群与胆汁酸谱的双向调控基础以及基于该轴的诊断和治疗策略进行综述,以期为相关研究和临床转化提供参考。

 

 Abstract: The gut microbiota and bile acids together constitute an important regulatory network within the gut-liver axis. After being synthesized in the liver, bile acids enter the intestine, where they undergo deconjugation, dehydroxylation, oxidation, and isomerization mediated by the gut microbiota, thereby forming a bile acid prof ile with diverse functional activities and biological ef fects. Conversely, bile acids infl uence the composition and function of the gut microbiota through their antimicrobial properties, receptor-mediated signaling, and regulatory ef fects on the intestinal barrier. In recent years, increasing research on the gut microbiota-bile acid axis has suggested that dysregulation of this axis not only participates in the development and progression of metabolic associated fatty liver disease, alcoholrelated liver disease, virus-related liver disease, cholestatic liver disease and hepatocellular carcinoma, but is also closely associated with infl ammatory activity, the degree of fibrosis, therapeutic response, and prognosis. Integrated analysis of bile acid prof iles and microbiota characteristics is expected to provide new insights into noninvasive stratif ication, dynamic monitoring, and precise intervention for liver diseases. This review focused on the basis of bidirectional regulation between the gut microbiota and bile acid prof iles,and axis-based diagnostic and therapeutic strategies, with the aim of providing a reference for related research and clinical translation.
