长链非编码RNA FAL1在肝细胞癌患者的表达及其与预后的相关性

摘要:目的 研究肝细胞癌患者癌组织与癌旁组织中长链非编码RNA FAL1(long non-coding RNA FAL1,lncRNA FAL1)表达差异及其与预后的关系。方法 回顾性选取2014年11月至2015年5月重庆市 九龙坡区中医院进行手术治疗的89例肝细胞癌患者为研究对象,术中留取患者的癌组织及距癌组织边 缘> 5 cm的癌旁组织标本。采用q-RT PCR检测各组织中lncRNA FAL1表达水平,观察lncRNA FAL1表 达水平与患者临床病理特征的关系。采用Kaplan-Meier生存曲线和Log-Rank检验分析lncRNA FAL1相对 表达量对患者预后的影响。通过COX多因素分析判别肝细胞癌预后的独立危险因素。结果 肝细胞癌患 者癌组织Inc RNA FAL1的表达水平显著高于癌旁组织,差异有统计学意义(t = 24.00,P < 0.001)。 以癌组织中lnc RNA FAL1表达水平的中位值1.70为界,分为高表达组(60例)和低表达组(29例), 两组患者的性别、年龄、TBil、ALT、吸烟史、肿瘤数目、肿瘤直径、是否感染肝炎病毒、是否患自 身免疫性肝炎等差异无统计学意义(P > 0.05)。lncRNA FAL1在有淋巴结转移、分化程度低、TNM 分期为Ⅲ~Ⅳ期、有门静脉癌栓、包膜完整、有肝硬化患者中的表达量高于无淋巴结转移、分化程度 高、TNM分期为Ⅰ~Ⅱ期、无门静脉癌栓、包膜不完整、无肝硬化患者,差异有统计学意义(P < 0.05)。Kaplan-Meier分析表明,lnc RNA FAL1高表达组患者3年内存活率(20.1%)显著低于低表达组 (61.2%),差异有统计学意义(χ 2 = 11.575,P = 0.001)。lnc RNA FAL1高表达是肝细胞癌预后的独 立危险因素(RR = 1.952,95%CI:0.627~2.106,P = 0.014)。结论 lncRNA FAL1在肝细胞癌患者癌 组织中高表达,是预测肝细胞癌预后的独立危险因素,可成为临床早期诊断与治疗的潜在靶标。
Abstract: Objective To investigate the expression of long non-coding RNA FAL1 (lnc RNA FAL1) in patients with hepatocellular carcinoma and its relationship with prognosis. Methods Total of 89 cases with HCC who underwent surgical treatment in Jiulongpo Hospital of Traditional Chinese Medicine from November 2014 to May 2015 were selected retrospectively. During the operation, the cancer tissues of the patients and the non- cancerous tissue specimens with a distance of longer than 5 cm from the edge of the cancer tissues were collected. The expression of lncRNA FAL1 in hepatocellular carcinoma tissues and para-cancerous tissues were detected by q-RT PCR, and the relationship between the expression of lnc RNA FAL1 and the clinicopathological features of the patients were observed. Kaplan-Meier survival curve and Log-Rank test were used to analyze the effects of relative expression of lnc RNA FAL1 on the prognosis of patients. Multivariate COX analysis was used to identify the independent risk factors of prognosis of hepatocellular carcinoma. Results The expression level of lnc RNA FAL1 in hepatocellular carcinoma tissue was much higher than that in para-cancerous tissue (t = 24.00, P < 0.001). According to the median value (1.70) of lnc RNA FAL1 expression in cancer tissues, the patients were divided into two groups: high expression group (60 cases) and low expression group (29 cases). There were no significant differences in sex, age, TBil, ALT, smoking history, tumor numbers, tumor diameter, hepatitis virus infection and autoimmune hepatitis between the two groups (P > 0.05). The expression levels of lncRNA FAL1 in patients with lymph node metastasis, low differentiation degree, TNM Ⅲ~Ⅳ phase, portal vein tumor thrombus, completed envelope and liver cirrhosis were higher than those of patients without lymph node metastasis, high differentiation degree, TNM Ⅰ~Ⅱ phase, without portal vein tumor thrombus, uncompleted envelope and without liver cirrhosis, the differences were statistically significant (P < 0.05). Kaplan-Meier analysis showed that the three-year survival rate (20.1%) in lnc RNA FAL1 high expression group was lower than that of the low expression group (61.2%), the difference was statistically significant (χ 2 = 11.575,P = 0.001). lncRNA FAL1 level was an independent risk factor for the prognosis of hepatocellular carcinoma (RR = 1.952,95%CI:0.627~2.106,P = 0.014). Conclusions lnc RNA FAL1 is highly expressed in hepatocellular carcinoma tissues, which is an independent risk factor for the prognosis of hepatocellular carcinoma, and can be a potential target for early diagnosis and treatment.
