慢性乙型肝炎患者外周血单个核细胞miRNA-122和miRNA-130a的表达及对肝脏炎症程度的预测价值

摘要:目的 探讨慢性乙型肝炎(chronic hepatitis B,CHB)患者外周血单个核细胞 微小RNA(microRNA,miRNA)-122和miRNA-130a的表达及对肝脏炎症程度的预 测价值。方法 选择衡水市第三人民医院2020年12月至2022年12月收治的81例CHB患 者为研究对象,依据肝脏炎症程度分为轻度组(32例)和中重度组(49例);另选 择同期健康体检者72例作为对照组。采用速率法测定天冬氨酸氨基转移酶(aspartate aminotransferase,AST)和丙氨酸氨基转移酶(alanine aminotransferase,ALT)水平, 采用化学氧化法测定总胆红素(total bilirubin,TBil)水平;采用实时荧光定量聚合酶 链反应测定miRNA-122和miRNA-130a的相对表达量。采用受试者工作特征(receiver operator characteristic,ROC)曲线分析miRNA-122和miRNA-130a对中重度CHB的预 测价值。结果 轻度组和中重度组CHB患者AST [(76.49 ± 23.51)U/L比(279.96 ± 73.53)U/L比(20.13 ± 6.37)U/L]、ALT [(98.94 ± 27.53)U/L比(248.82 ± 67.47)U/L比 (17.84 ± 5.31)U/L] 和TBil [(20.19 ± 3.57)μmol/L比(36.24 ± 6.52)μmol/L比(9.48 ± 1.79)μmol/L] 水平显著高于对照组,中重度组CHB患者AST、ALT和TBil水平显著 高于轻度组(P均< 0.05)。轻度组和中重度组CHB患者miRNA-122(1.72 ± 0.34比 3.27 ± 0.78比1.00 ± 0.01)和miRNA-130a(2.31 ± 0.54比4.25 ± 0.78比1.00 ± 0.02)相对 表达量显著高于对照组,中重度组CHB患者miRNA-122和miRNA-130a相对表达量显著 高于轻度组(P均< 0.05)。ROC曲线分析表明,miRNA-122预测中重度CHB的曲线下 面积为0.961,敏感度为87.80%,特异度为100.00%;miRNA-130a预测中重度CHB的曲 线下面积为0.984,敏感度为100.00%,特异度为91.10%。结论 CHB患者miRNA-122和 miRNA-130a呈高表达,且与肝脏炎症严重程度密切相关,中重度CHB患者miRNA-122 和miRNA-130a相对表达量高于轻度CHB患者。
Abstract: Objective To investigate the expression of microRNA (miRNA) - 122 and miRNA- 130a in peripheral blood mononuclear cells of patients with chronic hepatitis B (CHB) and their predictive value on the degree of liver inflammation. Methods Total of 81 patients with CHB in Hengshui Third People’s Hospital from December 2020 to December 2022 were selected and divided into mild group (32 cases) and moderate and severe group (49 cases) according to the severity of liver inflammation. In addition, 72 cases of physical examination of the same period were selected as control group. The levels of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) were measured by rate method and the level of total bilirubin (TBil) was measured by chemical oxidation method. The expression of miRNA-122 and miRNA-130a were determined by real time fluorescent quantitative polymerase chain reaction. Receiver operator characteristic (ROC) curve was used to analyze the predictive value of miRNA-122 and miRNA-130a on moderate and severe CHB. Results Levels of AST [(76.49 ± 23.51) U/L vs. (279.96 ± 73.53) U/L vs. (20.13 ± 6.37) U/L], ALT [(98.94 ± 27.53) U/L vs. (248.82 ± 67.47) U/L vs. (17.84 ± 5.31) U/L] and TBil [(20.19 ± 3.57) μmol/L vs. (36.24 ± 6.52) μmol/L vs. (9.48 ± 1.79) μmol/L] of patients in moderate and severe group were significantly higher than those of mild group, and the above indexes of patients in mild group were significantly higher than those of control group (all P < 0.05). The relative expression of miRNA-122 (1.72 ± 0.34 vs. 3.27 ± 0.78 vs. 1.00 ± 0.01) and miRNA-130a (2.31 ± 0.54 vs. 4.25 ± 0.78 vs. 1.00 ± 0.02) of patients in moderate and severe group were significantly higher than those of mild group, and the above indexes of patients in mild group were significantly higher than those of control group (all P < 0.05). ROC curve showed that the area under the curve of miRNA-122 on predicting moderate and severe CHB was 0.961, the sensitivity was 87.80% and specificity was 100.00%. The area under the curve of miRNA- 130a on predicting moderate and severe CHB was 0.984, the sensitivity was 100.00% and the specificity was 91.10%. Conclusions MiRNA-122 and miRNA-130a were highly expressed in CHB patients and closely related to the severity of liver inflammation. The relative expression levels of miRNA-122 and miRNA-130a in moderate and severe CHB group were higher than those of mild group.
